It takes just three easy steps
1. Provide Student Health Examination (HRS-H Form 3040) obtained from your doctor. **Must be current information (no more than one year old) or provide religious/conscience exemption waiver (a signed letter indicating you are taking the religious exemption). 2. Provide Florida Certificate of Immunization (HRS Form 680) **Must be current information (no more than one year old) or waiver form DH 681 (Religious Exemption from Immunizations Form). - obtained from local health department. 3. Completed UFVPS enrollment form - fill out form online ( see below) Note: Students are not enrolled until all information and documents are received and you receive an enrollment confirmation via email. Please make sure to accurately input your correct email address. (By listing your child's date of birth you are attesting to their legal age from an official birth certificate, subject to FL statutes)
The state requires students to be withdrawn from their previous school. If you don't your child will be marked truant and the authorities may knock on your door. Legally, you don't have to provide enrollment confirmation to another school before they can be withdrawn, but they may ask for and sometimes even demand it. We have found that it's easier for you, and they ask less questions, if you inform them you are going to home school your child. Whether you tell them anything depends on your personal choice. We can provide enrollment confirmation. Just get their name and email address and send it to us requesting it.
